Is Laser Liposuction The Affordable Liposuction Alternative?

Liposuction is a common surgical procedure that removes fat from particular areas of the body in order to improve contour and overall appearance. Some common areas that are operated on are the abdomen, thighs, arms, chin, neck, calves, back and hips although there are other areas as well. Traditional liposuction involves tubes being inserted underneath the skin to vacuum the fat cells out of the body. There are now new procedures that have made liposuction safer and have made recovery time much shorter.

One of the new techniques involves the use of laser technology. This technique dissolves the fat underneath the skin with less trauma to the skin and surrounding tissues. This leads to less swelling, bruising and overall discomfort. One benefit of this procedure is that it is ideal for treating fibrous fats found in the chin, ankles or man’s chest. Those hard to treat areas have been the cause of low confidence for many and now there is a way to treat them.

Laser Liposuction – Newer Body, Less Pain

Out of several liposuction techniques popular these days, Laser Liposuction – Miami is one of the most sought-after.   Laser lipolysis system offers a radical and minimally all-encompassing procedure for the taking away of fat.  Laser Lipo offers less downtime and side-effects than conventional liposuction because it uses a high-powered laser.

The Advantages of Laser Liposuction

Many people who are not overweight or obese have areas of extra fat on an otherwise fit body. No matter how hard we exercise and watch our diet, some areas of fat just won’t go away. These areas tend to be around the waist in men, and the tummy, hips and thighs in women. Especially after pregnancy, loose skin and tummy fat seem to persist even after extreme exercise and dieting.

Liposuction can remove these areas of extra fat in many people, and give a fresh start to a healthy diet and exercise program. However, traditional liposuction can be a painful experience and in many cases require a long recovery period, facts which dissuade many from having liposuction. Also, 20% of patients who have traditional old time liposuction have skin irregularities after the liposuction procedure.

I have been using tumescent laser liposuction with the new SlimLipo laser lipo technique for fat removal, laser body sculpting and laser liposculpture for almost a year now and I am amazed at how much better the tumescent laser liposuction technique is than the old time non-laser liposuction method. What Is Tumescent Laser Liposuction?

Tumescent laser liposuction is a new liposuction method that combines the use of tumescent solution with laser melting of fat before fat removal.

Tumescent solution is a liquid that contains an anesthetic or numbing solution and a vasoconstrictor, a medication that helps control bleeding. The tumescent solution is injected beneath the skin, into the fat to be removed, before the laser liposuction or laser lipo is started.

The tumescent solution numbs the skin area for pain control which can last up to 24 hours. The solution also prevents bleeding which reduces post laser liposuction bruising and swelling, which speeds recovery.
The tumescent solution is injected under pressure, which swells or distends the fat and makes removal easier. The distention and hydration (adding water to) of the fat also improves the effectiveness and safety of the laser treatment to dissolve the fat.

How Is the Tumescent Laser Liposuction Treatment Done?

Tumescent laser liposuction may be done with you awake or asleep. Many areas can be treated with you awake using only the local anesthesia provided by the tumescent solution. However, the benefits of tumescence are so important that tumescent solution is also used when laser liposuction is done under general anesthesia, for control of bleeding, improved fat removal and for post operative pain management.

After your skin has been cleansed with a sterile anti-bacterial soap, the skin area to be liposuctioned is sterilely draped to prevent contamination.

Tiny punctures are made in hidden areas of the skin, and a small needle inserted beneath the skin into the fat to be removed. The tumescent solution is then injected into the fat under pressure to swell the fat and deliver the anesthetic and vasoconstrictor (prevents bleeding) solution.

After a brief waiting period until your skin is numb and the tumescent fluid has taken effect, the laser fiber is inserted through the same puncture sites and the fat is melted with the laser. The fat is liquefied into a thin liquid which is much easier to remove than fat that is removed with traditional old time liposuction.

After the fat has been liquefied, a small tube called a cannula is inserted and the liquid fat removed by suction. Because the fat has been liquefied, much smaller cannulas can be used than with traditional non-laser liposuction. This fact results in a less traumatic procedure with tiny hidden scars, very different from old time liposuction.

The laser also tightens the skin as the fat is lased. The laser heats the fibers that attach your skin to your body, and causes these fibers to shorten which pulls the skin closer to the body. Microscopic studies have shown that many more fibers, called connective tissue, are left after laser liposuction than after non-laser liposuction. I believe that this fact helps explain the improved skin tightening and smooth skin after laser liposuction.

How Will I Feel After Tumescent Laser Liposuction?

Because the tumescent laser liposuction technique places numbing solution in the liposuction area, you will have very little discomfort immediately after and up to 12 hours after your laser liposuction procedure. In addition you will be given strong pain killers to manage any discomfort.

The second day you will feel discomfort in the treated areas, however in my experience, the level of discomfort is certainly much less than that experienced with traditional non-laser liposuction. By day 3 most patients are very comfortable.

You will be wearing a supportive compression garment on the treated areas which adds additional support and comfort.
You will be up and around, able to eat, walk, use the bathroom and do normal daily activities the day after your tumescent laser liposuction. I recommend that you not go to the gym or do vigorous physical activity for the first week to ten days because I want the healing process to be undisturbed during this time.

How Long Is the Recovery after Tumescent Laser Liposuction?

You will be active and walking from day one. However I will limit your vigorous exercise and weight lifting or aerobics for ten days.
Your garment will be changed to a light Spanks type garment after three to five days. If small sutures are used to close the small puncture sites they will be removed at day five.

When small areas like the chin or a minor tummy liposuction are done, some people return to work the next day. I do not recommend returning to work the same day-I want you to be quiet and rest the first day.

Recovery to a more normal skin appearance can take ten days to two weeks for swelling and bruising, if any, to resolve.

Your skin appearance, skin tightening and laser liposuction result will continue to improve for three to six months as the skin gradually tightens over the areas where fat has been removed. The remarkable thing about the tumescent laser liposuction technique is that the skin is much smoother after laser liposuction than after traditional old time, non-laser liposuction. I believe that the laser dissolving of the fat prior to removal results in a more uniform fat removal.

Which Laser is best for Tumescent Laser Liposuction?

There are several lasers in use for laser liposuction. I use the SlimLipo Aspire laser liposuction system made by Palomar Medical. I have chosen this laser because the SlimLipo uses the only laser wavelength which is specifically absorbed by fat. The SlimLipo uses a 924nm (nm=nanometers, one millionth of a meter and describes the wavelength of the laser color in the visible light spectrum, which for 924nm is yellow, the color of fat) laser wavelength.

The 924nm laser wavelength used in SlimLipo dissolves or releases three to five times as much fat during lasing as do the other commonly used wavelengths. The other wavelengths used by different laser liposuction machines target water, not fat, and are less efficient and less specific at fat dissolving.

The 924nm SlimLipo also works very well in hydrated fat, meaning that fat dissolving with the laser works very well after the tumescent fluid has been injected.

How Do I Find a Surgeon for My Tumescent Laser liposuction?

I am a board certified plastic surgeon, thus I am very prejudiced that you should have your tumescent laser liposuction performed by a board certified plastic surgeon. My view is that if you are going to have surgery of any kind, you want a board certified surgeon who has been fully trained and is expert at taking care of a surgical patient. You want the safety of knowing that if something goes wrong, your doctor will have the skill, experience and training to take care of you.

Some dermatologists do liposuction and are trained in liposuction and lasers. If you wish to have a dermatologist do your laser liposuction, I recommend that you make certain that your dermatologist is board certified and has had a fellowship in dermatologic surgery.

Do not be intimidated by the white coat and afraid to ask the doctor important questions. You are an educated consumer, and a good doctor will welcome the fact that you are prepared and have done your reading. I prefer to have informed patients, they always do better after surgery. Plastic surgery is no place for snap decisions or impulse buying.

• Are you certified by the American Board of Plastic Surgery?
• How often do you do tumescent laser liposuction?
• Do you think I am a good candidate for tumescent laser liposuction?
• Are you comfortable operating on me with my particular goals?
• Do you anticipate a good result-why?
• Can you show me before and after picture of patients that you have operated on?
• Can I speak to a patient who has had this surgery by you?
• What are the arrangements for an emergency during nights and weekends-are you available?
• Will my tumescent laser liposuction be done in an approved operating room or outpatient facility under standard sterile and safety precautions? Are emergency services and monitoring available if needed?

Most importantly, trust your gut feelings during your visit with the doctor. Is the office clean and professional? Do the doctor and staff treat you in a respectful manner? Ask yourself will I be comfortable calling these people and depending on them if I have an emergency at night or on weekends? Will I be comfortable expressing my concerns or disappointment if I feel that way? If you detect any discomfort or uncertainty, you should consult another doctor.

Tumescent laser liposuction is a remarkable, revolutionary advance in liposuction and fat removal techniques. The benefits can be very significant. However, the results of liposuction, like any surgical procedure are very dependent upon the skill and expertise of your surgeon. Choose wisely and I expect that you will have a very happy result.

Tumescent Laser Liposuction?Less Discomfort Quicker Recovery After Liposuction

Laser liposuction techniques have generated a great deal of attention recently and are rapidly increasing in popularity. However, there is very little research available to support claims that the technique is more effective than other liposuction methods.

PHILADELPHIA, May 26, 2010 – Laser liposuction is a relatively new technique which employs laser technology to remove stubborn pockets of fat that have been resistant to diet and exercise. Through savvy advertising by the major players in laser liposuction, the technique has garnered a great deal of attention and has developed a reputation for providing safer, more effective results. However, there has been very little research conducted which supports these assertions. As a result, many patients are opting to undergo laser liposuction based on claims that are not grounded in scientific evidence.

In 2006, SmartLipo™ became the first laser liposuction product to receive FDA approval. However, it received Section 510(k) approval, which is reserved for devices and technologies which are deemed substantially equivalent to pre-existing devices that have already been approved by the FDA. As a result, SmartLipo did not have to undergo the strict premarket approval application required of devices which have not been classified as substantially similar to existing technology. By skipping this strict approval process, SmartLipo and other laser liposuction creators were able to market their product to the public without generating a body of research to clinically prove its safety and effectiveness.

While many plastic surgeons have started using laser liposuction technology with their patients, there is still a substantial portion professionals in the field who are not convinced that this technique is more effective than traditional liposuction methods.

“Laser liposuction has received a lot of attention in the media recently claiming superior results to standard tumescent techniques and other techniques such as ultrasonic liposuction,” says Dr. David Bottger, a board certified plastic surgeon practicing in Philadelphia. “The implication is that laser liposuction produces more skin tightening than the other techniques. In fact, there is no scientific evidence that this is the case. Additionally, laser liposuction is associated with an increased incidence of seromas (fluid collections), skin burns, and skin retraction. Standard tumescent technique continues to produce safe, predictable, and excellent results.”

A review of the SmartLipo website supports Dr. Bottger’s assertion that these claims lack scientific merit. While the SmartLipo website makes frequent claims about safety and exceptional results, it does not reference any clinical studies to validate these claims. Dr. Bottger urges patients considering liposuction to thoroughly research a variety of techniques and discuss these options in detail with their plastic surgeon before deciding on the appropriate method for their needs. What Causes Belly Fat and Fat Stomach

The several causes of belly fat or a fat tummy or pot belly are:

- Intra-abdominal Fat-Fat serves a very important survival function. Fat stores energy to provide nutrition during periods when we do not have adequate food intake. Thus fat is a survival mechanism during periods of starvation. Fat is stored inside the abdomen in a structure called the mesentery. The mesentery is wrapped around our intestines and supports the intestines. Huge amounts of fat can be present in the mesentery when we are overweight or obese. When there is excess intra-abdominal fat in the mesentery, the fat actually pushes out and stretches the muscles of our abdominal wall and creates a fat belly. Only dieting or caloric restriction can cause the intra-abdominal fat to be used for energy production and disappear.

- Abdominal Wall Laxity-The abdominal muscles (abdominal wall), called the abs, hold our intestines and abdominal contents in the abdomen. These muscles can become stretched and loose after extensive weight gain, pregnancy, and inactivity. When the abdominal muscles, or abdominal wall loses tone, becomes stretched or lax, the contents of the inside of the abdomen push the abdominal wall forward and create a fat tummy or pot belly. Tightening of the abdominal wall, that is restoring the tone of the abdominal or tummy muscles can only be achieved with ab exercises and conditioning of the tummy muscles or with abdominoplsty or tummy tuck surgery. Stomach Skin Tightening Exercises

- Subcutaneous Fat beneath the Belly Skin- The fat directly beneath the skin on the belly also stores fat. This fat is called the subcutaneous (beneath the skin) fat and is normally about 1/2 inch thick in most people. When we gain weight and have excess fat, this subcutaneous fat layer becomes thicker, sometimes up to 4 or 5 inches thick. Laser liposuction is a very effective technique to remove subcutaneous fat. The heat from the laser melts the fat into a thin liquid which is easily removed by suction through very small tubes called cannulas.

- Loose Belly Skin-Multiple pregnancies and repeated weight gain stretch the abdominal skin and create loose belly or tummy skin. The major breakthrough in laser liposuction is the ability of the laser to tighten stomach skin. Previous liposuction techniques removed fat but left loose belly or tummy skin after fat removal. With laser liposuction, the heat from the laser tightens the skin during the laser fat dissolving process.

Laser Liposuction to Lose Belly Fat and Flatten the Stomach

Because the laser liposuction fiber dissolves fat into a thin liquid, removal of fat is much easier. This allows the surgeon to use very tiny tubes or cannulas to suction the fat out of the tummy skin. As a result very tiny holes, usually placed inside the belly button, are all that is required. This means very tiny, almost invisible scars. In addition, in my experience, following laser liposuction the skin is left smooth without the lumps, bumps and contour deformities often seen with traditional liposuction.

Skin tightening following this new technique has been truly remarkable in my experience. In patients who are fit and not overweight, I have been able to do laser body sculpting to actually reveal the outline of the abdominal muscles after healing. In the words of one of my patients “I got my six pack back.”

My patients seem to recover more quickly after this new procedure. Many say they fell fine the day after the procedure. However, I ask my patients not to return to vigorous or strenuous activity or work outs for at least two weeks.

Laser liposuction, known as Smart Lipo, is already in use in Italy, Australia and the Far East and will soon be available at British cosmetic surgery clinics. Smart Lipo, which takes place under local anaesthetic, also claims to be able to smooth away double chins, love handles and flabby upper arms or ‘batwings’. The firm marketing the equipment in the UK claims that the technique has already been used safely on thousands of men and women around the world.

However, some experts are questioning the effectiveness of the procedure and they are warning that no type of cosmetic surgery is without some risk. There are also worries that speeding up the surgery into so-called “lunch break lipo” could trivialise cosmetic surgery. The problem, they say, is that patients may rush into surgery without fully understanding the risks. Only last year an alternative fat-dissolving procedure, known as Lipostabil, was banned by the medicines’ watchdog, the Medicines and Healthcare Products Regulatory Agency. These injections, known as “flab jabs” will no longer be used, pending further research into their safety.

Smart Lipo uses a tiny laser to break up the membrane of fat cells. This allows the oil stored inside the cells to seep out and be broken down by the liver. Collagen production is stimulated, causing the skin to tighten and bruising is minimal. This is in contrast to conventional liposuction, which uses a hollow needle to suction fat out of the body. Whilst a local anaesthetic can be used for small areas, treatment of larger areas such as the hips and thighs usually requires a general anaesthetic and an overnight stay.

It is considered that Smart Lipo will mostly appeal to people who want small amounts of fat to be removed, but do not necessarily want to go into hospital or undergo a major procedure involving general anaesthesia and an overnight stay. The best results will be for those people of normal or just above normal weight, who have localised stubborn fatty areas. It is clear that those who want large areas of fat removed are unlikely to get the results they need during a lunch break treatment.

The number of patients undergoing liposuction surgery is steadily rising, mainly due to the safer and less painful alternative techniques on offer. Although, as with every surgical procedure, there will always be at least a slight element of risk, it can be said that everything we do in life involves a risk, such as going out of the house in the mornings, walking across a busy road or driving along the motorway. Laser liposuction, a European technology of assisted liposuction, is a procedure that reduces the pain, swelling, and bruising that are sometimes associated with traditional methods of liposuction.

Liposuction: The Basics

Laser liposuction or traditional liposuction? It’s a question that many potential patients ask themselves. They do their research. They see the differences between the two methods. Discussing Laser Liposuction?

Laser liposuction is the hottest thing in liposuction that we have seen for a number of years. In the U.S. this has been available for approximately 2 years. Smartlipo was the first on the scene and was FDA approved in 2007. Smartlipo is certainly still the largest in this race but a few newcomers including slimlipo, LipoLite, lipotherme and cool lipo to name a few have come along.

Liposuction has been around for years. Major advances in techniques have occurred. The most significant advances are the development of tumescent technique (injection of numbing solution) and the use of smaller cannulas. Is laser liposuction the next great advancement in liposuction?

What is laser liposuction?

There is a general misunderstanding about laser liposuction. Most advertising would suggest that smartlipo and its competitors have completely reinvented liposuction. In reality all of the basic steps involved with traditional liposuction are performed including:
1. Sedation of the patient
2. Injection of tumescent fluid (numbing solution)
3. Suction of fat.
With laser lipolysis there is an additional step which involves the burning of fat just before removal.
Initially, the laser was intended to be used without liposuction of the fat. ‘Melted’ fat was to be resorbed by the body. This has generally been shown to be ineffective. The fat must be removed after the burning, this is achieved in the EXACT same manner as would be performed without the laser.

The claimed advantages of burning the fat are:

1. Skin tightening
2. Less pain
3. Quicker recovery
4. Smoother results
5. Less invasive procedure, some ads even call it fat removal “without surgery or scars”
6. Quicker procedure

How accurate are these claims?

According the the American Society of Plastic Surgeons, “The only study that has scientifically compared this to standard liposuction has found no differences between these techniques in terms of the results.”

The author (Dr. York Yates) is a plastic surgeon in the Salt Lake City, Utah area.

“I have been excited about the possibility of additional skin tightening with laser liposuction. This is often a problem in older people or folks who have lost a lot of weight.”

“I was recently given the opportunity to ‘try out’ a laser lipo machine on a patient we had scheduled for liposuction. She had liposuction on the abdomen,upper hips, back, and inner thighs. The laser lipo was used on the abdomen to hopefully tighten some loose skin.”

“The experience was less than satisfying. There were not any complications due to the laser, however it just didn’t live up to the hype. Although the other treatment areas were nearly painless, the abdomen (laser treated) was extremely painful and was reported as a ‘burning’ sensation. The addition of the laser did not increase the amount of fat that could be removed, nor did I see any skin tightening.”

“The only difference I can see between the two procedures is increased pain, increased cost and need for deeper sedation. It is certainly just as invasive as traditional liposuction if not more so. I have researched this extensively and cannot find any ground to support any of the claims that laser lipo is making. Hopefully these studies are done and hopefully the results are favorable for skin shrinkage.”

Disadvantages of laser liposuction:

The purported advantages of laser liposuction don’t seem to be accurate. There are, however a few disadvantages of laser liposuction in the authors opinion:

1. Most surgeons (not all) using laser liposuction are not experienced plastic surgeons. “Cosmetic surgeon”, is not the same thing and may be an oral surgeon, family practice doctor, OB, radiologist or anyone with a medical degree.
2. The burning is painful
3. The burning requires a longer procedure with more time and risk. Does it make any sense to burn tissue before you remove it?
4. Burning of fat makes it a more (not less) invasive procedure
5. The procedure is more expensive (the equipment is very expensive)

How has laser liposuction gotten so popular if it doesn’t do what it says it does?

Purely marketing genius. Add a laser to anything in medicine and the assumption is that it must be better. This undoubtedly true in Lasik eye surgery and laser hair removal. The bombardment of billboards, TV and print media have made it extremely popular. Many of these ads have been misleading and irresponsible. The claims make laser lipolysis sound like a quick lunch hour procedure. It is simply not the case. Liposuction is one of the most dangerous procedures that plastic surgeons do and all plastic surgeons take it very seriously. The fact that the procedure can be done with light sedation and the incisions are small should not be misinterpreted as minimally invasive. The majority of the deaths (although very rare) in cosmetic surgery are liposuction related.

Many of the doctors using these devices learn to do liposuction over a weekend course, a craft that takes plastic surgeons 6-7 years. The consumer should be very careful. Find the right doctor and together determine what would be your best treatment option. The technology behind the doctor is less important when it comes to liposuction.
