Fat deposits in the abdominal region tend to be the ones that resist the effects of exercise and diet.</p>
<p>Most patients who opt for abdominal liposuction have exhausted their attempts to lose that fat with dieting, training and exercise. Abdominal fat, or visceral fat, does not respond the way other fat does, when you diet and work out. But it does respond well to abdominal liposuction, so patients are usually very pleased at the results of this procedure. To add to this, the effects achieved with this liposuction are permanent, barring unusual circumstances.</p>
<p>You need only be subjected to a local anesthetic for the tumescent technique, so this eliminates the worries and risks and cost associated with general anesthesia. The trauma brought to the fatty tissue, and the bleeding caused are reduced dramatically with the tumescent liposuction technique. And most patients who have abdominal liposuction with the newer technique can be back to work in a few days.</p>
<p>Fat tends to localize itself in the abdominal area, and it is resistant to a lot of the exercise and diet that you may subject your body to, in trying to lose it. But this area responds well to liposuction. This is the reason that many men and women opt for abdominal liposuction to gain a flatter stomach.</p>
<p>The fat in your abdominal area can be intra-abdominal (internal) or subcutaneous (right under the skin). If you suffer from the internal fat, it can’t be removed by liposuction, because it’s too far inside the abdomen. Subcutaneous fat responds readily to liposuction, since it’s above the abdominal muscles. If your fatty look comes from bulging abdominal muscles, liposuction won’t help you, because that isn’t tissue that can be suctioned out.</p>
<p>You can test yourself to determine if you have the type of fat that will respond to abdominal liposuction. If you are able to grab a fold of your skin between your fingers and thumb, and it feels soft and is easily moved up and down, this is likely a fat deposit that will be amenable to liposuction.</p>
<p>Traditionally, the tummy tuck was the procedure of choice, but this is being overshadowed now by the acceptance of abdominal liposuction. The tumescent procedure uses small cannulas, and can give you the look of a tummy tuck without the invasiveness. If it’s determined that you still need a tummy tuck, you need only wait a time after the liposuction, and then it can still be done. But the success rate of abdominal liposuction is such that subsequent tummy tucks are not often needed anymore.</p>
<p>Tumescent liposuction is performed while the patient is only under a local anesthetic, so the risks and side effects are greatly decreased. Your abdominal area will be anesthetized and the incisions made are very small. Fatty deposits are removed from all different directions, to insure that the results will be even.</p>
<p>After your abdominal liposuction, you are usually able to get right up and walk. The surgeon will apply a mild compression garment and dressings after the procedure is finished. You will be given detailed written and verbal instructions to follow. You will be told what to expect up-front, and this will ensure that you won’t have much difficulty or worry after the procedure is finished. <p>Tumescent Liposuction</p>
<p>Developed about 15 years ago, this is the most common, and safest, method. The term &#8220;tumescence&#8221; refers to the swelling and firming of fatty tissues by the means of infusing fluid into the tissue. After tiny incisions are made where the patient’s skin covers fatty tissue, the area is soaked with a saline solution that contains an anesthetic (Lidocaine) and adrenaline to reduce blood loss and bruising. This reduction of blood loss through the shrinking of capillaries has been a great advance, enabling plastic surgeons to remove much fat with little blood loss.Tumescent liposuction involves swelling the areas to be suctioned with a solution that equals two to three times the amount of the fat and fluid that will be suctioned out. In the Super-Wet technique however, the tissues are infused with an amount of fluid that equals the fat and fluids that will be suctioned. Super-wet involves the use smaller amounts of, or no, Lidocaine, thereby reducing the risk of Lidocaine toxicity.</p>
<p>Once the area has been sufficiently plumped up, the surgeon removes these fat cells through the insertion of a cannula, a tiny wand like instrument that suctions and removes the loosened fat cells via a high-power suction machine. The fat then glides through transparent vacuum-like tubes as fat and fluid are suctioned from your body. Because of the fluid loss, you need to be monitored carefully by an experienced anesthesiologist who can balance your fluid intake and output throughout the procedure.</p>
<p>There is nothing arbitrary about this process. A skilled surgeon, like a sculptor, has to use his sense of sight and touch to assess the areas of fat buildup and to contour and sculpt these areas into an ideal form. It also requires a great deal of arm movement and exertion by the surgeon, as he or she both suctions and sculpts. The tumescent technique has a low complication rate in and of itself, yet is also used as preparation for other kinds of liposuction techniques.</p>
<p>Power Assisted Liposuction</p>
<p>In essence, this is <a rel="nofollow" onclick="javascript:_gaq.push(['_trackPageview', '/outgoing/article_exit_link']);" href="http://www.beautifulself.com/lipo_technique.htm" target="_new">liposuction</a> hastened by a vibrating suction cannula. Developed by a German surgeon, this technique employs an electric-powered cannula, which like a jackhammer sucks out the fat faster. In my opinion, while it might help the surgeon to do his or her job faster, it takes away from the surgeon’s ability to feel the contour and the topography of tissue as this machine vibrates. Also, this machine might not be safe to be used in delicate or potentially dangerous areas such as the neck and face.</p>
<p>Ultrasonic Assisted Liposuction (UAL)</p>
<p>This method was first used in the 1990’s, and was considered an advance in treating especially fibrous areas, such as the male chests, back, and flanks. It uses cannulas that bombard subcutaneous fatty tissues with waves of ultrasound, thereby liquefying them through ultrasound energy and heat. An external method also delivers this energy by applying a paddle-shaped instrument to the skin. The higher general complication rate of this technique as compared to other conventional techniques has recently drawn surgeons away from this fad. Specifically, ultrasonic assisted lipoplasty (UAL) carries an increased risk of skin and tissue burns, permanent tissue damage and scarring, nerve damage causing lasting skin irritation or pain, and fluid buildup or formation of seroma – cavities of fluid beneath the skin. Because there is insufficient proof of the safety of UAL devices, the Food &amp; Drug Administration (FDA) has never approved the marketing and advertising of UAL devices.</p>
<p>What is Large Volume Liposuction?</p>
<p>What person today has not heard of gastric bypass surgery, more commonly known as &#8220;stomach stapling?&#8221; The news stories about musicians, new anchors and other celebrities who have undergone this procedure are frequent. Yet, only twenty years ago, the mere mention of gastric bypass would have elicited a declaration of &#8220;that would be malpractice!&#8221; In much the same way, our assessments about the safety of <a rel="nofollow" onclick="javascript:_gaq.push(['_trackPageview', '/outgoing/article_exit_link']);" href="http://www.beautifulself.com/lipo_risks.htm">Large Volume Liposuction</a> are progressing.</p>
<p>Notions of what is attractive are constantly evolving. The voluptuous actresses of the 1950’s would today be considered overweight. However, while we can’t really define what an &#8220;ideal&#8221; weight is, it is widely accepted that a significant portion of Americans today are overweight. This is both a health issue, and a self-esteem issue. More than half of American women wear a size 10 or more, and are unhappy with their size.</p>
<p>Just as the safety of gastric bypass surgery was once questioned, so is the acceptability of Large Volume Liposuction. However, in the hands of a skilled and experienced plastic surgeon who takes all of the usual precautions that are observed during any major surgery, a patient can successfully and safely have large amounts of fat removed.</p>
<p>When approximately two liters of fat (about the size of a Coca-Cola bottle) are removed, this is considered Small Volume Liposuction. This is successful when smaller localized &#8220;problem&#8221; areas of the body are being contoured. However, the removal of this quantity of fat will not make a difference in a large person whose goal is significant size reduction. In the latter case, large volumes of fat, five or more liters are removed, with the dramatic outcomes- 2 to 5 size reduction in cloth size. The surgeon removes the fat with an eye towards contouring the area as well. In this procedure, it is a good practice to combine the Tumescent with the Super-Wet Technique, as the latter does not use Lidocaine (or uses less), thereby avoiding the risk of Lidocaine toxicity.</p>
<p>If you are considering liposuction you should seek a plastic surgeon who has very extensive experience in performing this procedure in an optimal surgical facility. A minority of qualified plastic surgeons specializes in this.</p>

<p>At its simplist, liposuction is a two-steo process for removing fat. A hollow tube (cannula) is put under the skin, the tube is moved back and forth until the fat is dislodged, and the attached vacuum source draws the fat back through the cannula into the tubing and finally into the plastic bottle.</p>
<p>&#13;</p>
<p>Despite this apparent simplicity, there is more science to it than that. There are three interesting components about liposuction, all of which contribute to its safety and effectiveness. These include the concept of tumescence, water vaporization and suction, and tubing size and design.</p>
<p>&#13;</p>
<p>An integral part of the oepration is the placing of fluid into the areas where the fat will be suctioned. I frequently get asked by patients if I do tumescent liposuction. Most patients think that tumescence is a special liposuction technique when, in fact, it is part of every liposuction procedure. Putting large amounts of fluid into the fat areas to be suctioned beforehand is known as tumescent infiltration. This achieves two fundamental things, substantially reduces bleeding (from the fat being broken up by the liposuction tube and it distends the fat compartments which make the tube easier to pass through the fat being suctioned. Without tumescent infiltration, liposuction would quite bloody, give patients more pain, and have them bruised for a month, if not longer.</p>
<p>&#13;</p>
<p>The amount of suction generated by the liposuction machine, in short, makes liposuction possible. At the accepted amount of suction needed for liposuction (-20 cms of water or -1 atmosphere of pressure), the pulling of fat through the tubing certainly occurs. But it also causes the vaporization of water. If you have ever witnessed an actual liposuction procedure, you may have seen bubbles in the fat or bubbling in the plastic cannister. To some degree, this is actually water boiling&#8230;.or the vacuum pressure exceeding the vapor pressure of the water in the fat being removed. It is this vaporization that makes the viscosity (how thick it is) of fat, which is actually a combination of fat, blood, and infiltration fluid, less to improve its ease of flow through the tubing. From a flow standpoint, water that is vaporized ( a gas) flows 100 times faster than liquid water. This is also why liposuction done at higher altitudes, where the water vapor of pressure is less, is somewhat easier to do. (takes less suction from the machine)</p>
<p>&#13;</p>
<p>The tube (cannula) that is actually used under the skin differs in diameter and the holes at the tip. In days gone by, the cannulas were quite large but left a lot of depressions in the overlying skin after due to removing too much fat too fast and in large pieces. Today, small cannulas are used which removes fat more carefully and in smaller pieces, decreasing the problem of skin irregularities after surgery. The tip of the cannula is rounded so that it travels through the fat easier with less chance of penetrating something you shouldn&#8217;t. The holes at the tip are where fat sticks to and then gets sucked into the cannula. The more holes there are at the tip achieves two effects; increases the shearing effect (like a blade) on fat and makes more cross-sectional area through which fat can be suctioned.</p>
<p>&#13;</p>
<p>Currently, there are newer methods of liposuction or to be accurate, methods of loosening up the fat. These include ultrasonic and laser-assisted. While they sound quite advanced (and they are), there is no convincing evidence at this time that they are actually better than traditional liposuction. Better meaning&#8230;.you get more fat oout, smoother skin, and heal faster. Manufacturers market them as such but no study has yet proven it. And suction is still required to pull out loosened fat.</p>

The types of liposuction currently being performed are:</p>

Why is fluid injected? </p>
<p>&#13;<br />
Mixed with fluid, the fat suctions out more quickly and smoothly. There has always been the question of: How much fluid? In tumescent liposuction, a larger amount is injected, as much as three or four times the amount of fat to be removed. All this fluid expands the area, creating more space for the cannula movements.</p>

The cannula is used to break up the fat cells. Breaking their surfaces emulsifies (liquefies) the fat, again making it easier to remove. <p>If you&#8217;ve ever heard about liposuction or its technique, you&#8217;ve probably heard of tumescent liposuction. Body-Jet liposuction was developed from the tumescent technique, allowing this new procedure to bring about better results.</p>

<p>Why Body-Jet is safer and better</p>
<p>&#13;</p>
<p>The Body-Jet, also referred to as Water Jet Liposuction (WAL), uses a thin, fan-shaped jet that is channeled into the fat tissue. As a result, the fat easily loses its structure and is released.</p>

<p>With the Body-Jet, the patient will be prepared as though for a typical liposuction treatment. Anesthetization and vasoconstriction take place over a shorter period of time, compared with the tumescent technique.</p>

<p>The patient will be awake during the process and can work with the surgeon to obtain the desired result. The patient can get up during the treatment and look in the mirror to see the results. This is another advantage of the Body-Jet, since it gives the patient the ability to see the results, even before the process has ended.</p>

<p>Liposuction has become a popular way to get rid of excess fat today, with a number of procedures to choose from that can be tailored to each patient&#8217;s unique preferences and situation. However, the many choices available can also make it difficult to know which procedure will be the most effective, putting the patient at a distinct disadvantage. While the plastic surgeon of choice will be the best one to recommend a specific liposuction treatment, it is helpful if the patient has some basic information about the options before making the appointment for the initial consultation.</p>
<p><strong>Tumescent Liposuction</strong></p>
<p>One type of  liposuction that a patient may come across is known as tumescent liposuction. The term &#8220;tumescent&#8221; means firm and swollen, which is what happens when the tissue that is targeted for removal is infused with a solution during this type of treatment. The solution used will often include a local anesthetic, making the need for a general anesthetic during this procedure obsolete. While some say that this technique makes the fat removal process a bit easier, other surgeons complain that the swollen tissue makes it more challenging to effectively sculpt the body&#8217;s contour. This procedure takes more time, so it is rarely done in a hospital setting. However, it is well suited to a clinic, where tumescent liposuction is most often performed.</p>
<p><strong>The &#8220;Wet&#8221; Process</strong></p>
<p>The liposuction known as the &#8220;wet&#8221; technique involves injecting fluid into the treatment site as well. The amount of solution is considerably less than what is used with the tumescent liposuction process and may or may not include anesthetic in the mix. With this technique, a small percentage of what is suctioned out may be blood. This is significant, since too much blood getting taken during the procedure may result in a need for a transfusion. While the &#8220;wet&#8221; process removes a small percentage of blood, it is still more than other techniques like tumescent liposuction might involve.</p>
<p><strong>The &#8220;Super-Wet&#8221; Process</strong></p>
<p>As the name implies, this type of  liposuction will inject greater amount of fluid into the treatment area so that less blood is removed during the procedure. In this case, the amount of fluid that is injected into the tissue will be roughly the same as the fat taken out. Since this technique cannot infuse as much anesthetic into the area as the tumescent liposuction process, a general anesthetic is also used much of the time for this type of  liposuction. This is another important consideration, since general anesthetic can carry significantly more risks and side effects than a local might.</p>
<p>Liposuction can be the perfect cosmetic treatment for reducing pockets of fat on the body that have not responded significantly to diet and exercise. By choosing the best technique for each individual situation, the odds are greatly increased that the liposuction treatment chosen will be safe and effective for the patient. When a patient is well educated about the choices available, she and her doctor can partner together to find the best treatment for successful results.</p>

			<content:encoded><![CDATA[<p><strong>Traditional Liposuction &#8211; A Thing of the Past?</strong></p>
<p>Liposuction is a technology which is consistently evolving, encompassing many new methods, tools and combinations of previous methods to provide satisfying results for patients. In fact, there are so many new variations of liposuction and lipolysis that some medical professionals have speculated about a coming decline in traditional tumescent liposuction. While a wave of newer, less invasive techniques have been introduced in recent years, there are many plastic surgeons who still favor traditional techniques, with or without the assistance of power, laser, or ultrasonic cannula devices.</p>
<p>Traditional Liposuction</p>
<p>When surgeons refer to &#8220;traditional&#8221; <a rel="nofollow" onclick="javascript:_gaq.push(['_trackPageview', '/outgoing/article_exit_link']);" href="http://www.psplasticsurgery.com/plastic-surgery/liposuction.cfm">liposuction</a>, they are generally referring to the tumescent technique without the addition of adjunct methods such as ultrasonic assistance (UAL) or power assistance (PAL). To begin a tumescent procedure, the area to be treated is infused with a solution of saline and local anesthetic to decrease discomfort, constrict the blood vessels to limit bleeding and bruising, and engorge the fat cells, making them easier to remove from surrounding tissue. After the tumescent solution has been administered, a cannula (a thin hollow tube inserted below the skin) is used to manually break free the fat and then suction it away.</p>
<p>Ultrasound-Assisted Liposuction (UAL)</p>
<p>UAL adds another step to the tumescent liposuction procedure; a cannula with an ultrasonic tip is used to disrupt the fat cells, liquefying them for easy removal after the area is treated with the tumescent fluid. The high frequency ultrasound does most of the work the surgeon would have to do manually, while also pressing the surrounding connective tissue which is important in obtaining optimal skin retraction.</p>
<p>Power-Assisted Liposuction (PAL)</p>
<p>Another derivative method of tumescent liposuction is power-assisted liposuction (PAL). With this technique, an electric motor attached to the cannula. The motor produces rapid and steady motion of the cannula, which results in the dislodging of fat cells.</p>
<p>The Benefits of Current Methods of Liposuction</p>
<p>While traditional liposuction methods are invasive, meaning that a surgeon must make incisions in the patient&#8217;s body and the fat is actually suctioned out of the body, the technology has significantly advanced to minimize surrounding tissue damage and post-surgical recovery time. Patients experience shorter, more efficient surgeries with dramatic, immediate and lasting results. </p>
<p>However, since there is still a surgical component involved, plastic surgeons, along with medical device manufacturers, have been in search of new methods to achieve <a rel="nofollow" onclick="javascript:_gaq.push(['_trackPageview', '/outgoing/article_exit_link']);" href="http://www.psplasticsurgery.com/plastic-surgery/body-contouring.cfm">body contouring </a>without the need for surgery. As technology moves the field away from surgical procedures, the question remains: will this new technology make traditional liposuction obsolete?</p>
<p>The Bridge between Old and New: Laser Lipolysis</p>
<p>Introduced several years ago and continually being refined, laser liposuction uses the energy of laser light to break up fat cells. This approach can be used on its own with very small incisions (achieving only lipolysis, or destruction of fat cells) or in conjunction with aspiration of the laser treated area for immediate fat removal. When used only as a method of lipolysis, the ruptured fat cells are left in the patient&#8217;s body to be eliminated through the body&#8217;s natural waste-removal process.</p>
<p>One of the more popular names in laser lipolysis is Smartlipo™. Surgeons across the country are using this as a treatment for smaller areas such as the area beneath the chin where traditional approaches can be too aggressive. Laser liposuction techniques utilize very fine laser cannulas which offer exceptional control of the lipolysis process, resulting in smoother contours and enhanced skin retraction. </p>
<p>The Alternatives to Traditional Liposuction</p>
<p>Three non-invasive body contouring technologies that have been introduced in recent years are ZERONA®, which uses a low-level laser to target and destroy fat cells; LipoSonix™, which uses focused ultrasonic energy to target fat cells below the skin; and CoolSculpting™ by ZELTIQ, which uses selective cooling technology to kill fat cells. As of this writing, not all of these technologies have received FDA market clearance in the United States, but all are expected to become available for a range of applications in many markets in the coming years.</p>
<p>Each of these technologies achieves lipolysis, and each one is non-invasive, meaning that no incisions need to be made in order to destroy the fat cells. Understandably, this leads to faster recovery. Additionally, many of these new forms of lipolysis can take less time, but may require multiple treatments.</p>
<p>While these technologies seem promising for many patients, the results are not necessarily better than, or even equal to the results that can be achieved through tumescent liposuction. For example, more substantial reduction of fat volume simply is not yet possible with these devices, which emphasize more subtle results.</p>
<p>There needs to be more study of these devices and likely further technological improvements before they will begin to approximate the results achieved through surgical liposuction methods today. Of all the devices currently being reviewed, LipoSonix may become the most viable technology, since ultrasound energy has been studied more thoroughly and been is proven to be effective. But the jury is still  out  LipoSonix, which has a U.S. corporate headquarters near Seattle. The current methods of liposuction still give more dramatic results than non-invasive treatments.</p>
<p>Nevertheless, these new technologies are in their infancy, meaning that there is potential for non-invasive approaches to become a major method for body contouring as time goes on. But as of the year 2010, traditional liposuction certainly has not been made obsolete just yet.</p>

The anesthetic causes the compartments of fat to become swollen and firm or “tumesced.” The expanded fat compartments allow the liposuction cannula to travel smoothly beneath the skin as the fat is removed. This type of liposuction, called tumescent liposuction, can be used on any area of the body but is most often used on areas that require careful precision such as the face, neck, arms calves and ankles. Both men and women of all ages have been seeking tumescent liposuction.</p>

<p>This type of liposuction is a fairly new method that reduces post-operative bruising, swelling and pain. The amount of blood loss is minimized during tumescent liposuction, and this technique also reduces the chances that a blood transfusion will be necessary during the procedure. Liposuction patients are very glad to hear that.</p>

<p>Although blood loss is minimized with tumescent liposuction, patients undergoing the procedure still face the same risks associated with traditional liposuction. Some of the risks with this type of liposuction include pulmonary edema (fluid in the lungs) and lidocaine toxicity (too much lidocaine).</p>

<p>General anesthesia is typically the best option for the liposuction procedure. Or, if it’s appropriate, the tumescent solution may serve as the only means of anesthesia for the procedure. Your cosmetic surgeon will discuss all anesthesia options with you at the time of your consultation.</p>

<p>The Process</p>
<p>&#13;</p>
<p>In tumescent liposuction, the warmed tumescent liquid (a mixture containing lidocaine, epinephrine and intravenous fluid) is injected into the area to be treated. As this liquid enters the fat, it becomes swollen, firm and blanched. Liposuction is then performed on the tumesced areas.</p>
<p>&#13;</p>
<p>The anesthesia used can help to provide pain relief following liposuction and decrease the need for additional pain meds. For the first couple of days, most patients experience swelling in the areas that were treated and some fluid drainage from the incision sites. Patients usually resume light physical activity within the first few days following surgery, and normal physical activity can be resumed within a few weeks.</p>

<p>Patients typically see a marked difference almost immediately after surgery. After three weeks, more improvement can be seen after the majority of the swelling has subsided. After approximately three months, any persistent swelling will disappear and your final shape and contour will be visible.</p>

<p>Tumescent liposuction involves the surgical removal of excess body fat using a combination of traditional liposuction techniques and tumescent anesthesia. The procedure is relatively new to the cosmetic surgery scene, having come into widespread use in the 1990&#8242;s. Tumescent liposuction offers significant benefits over traditional liposuction techniques for its ability to minimize post-operative discomfort, curtail surgical bleeding, and reduce bruising and swelling during the recovery period. </p>
<p>Tumescent liposuction is unique in that it uses large volumes of a dilute solution of lidocaine, paired with the drug epinephrine. The lidocaine serves as a local anesthetic, eliminating the need for patients to go under general anesthesia. The epinephrine serves to temporarily restrict capillaries, thereby reducing bleeding during the surgery. This combination allows surgeons to perform liposuction using minimal amounts of sedatives. The lidocaine is retained in the body for several hours after the procedure, giving the patient up to 12 to18+ hours of pain-free recovery. The use of tumescent anesthetic also eliminates the drunken feeling felt by many patients after going under general anesthesia, as well as the nausea felt by traditional liposuction patients.</p>
<p>At the start of the procedure, the surgeon anesthetizes the skin at select locations using tiny needles. Once the skin has been numbed, longer needles are used to anesthetize the subcutaneous fat with large quantities of the dilute lidocaine and epinephrine solution. <p>Tumescent liposuction, as with any other type of surgical procedure, does carry some risks. Patients should expect some side-effects from the procedure such as bruising, swelling, and temporary numbness of the treated areas. Significant drainage will occur in areas where adits were formed. Some skin irregularities may result from the procedure, though this side-effect is greatly reduced with the use of both the tumescent anesthesia and microcannulas.
