<p>Tumescent liposuction involves the surgical removal of excess body fat using a combination of traditional liposuction techniques and tumescent anesthesia. The procedure is relatively new to the cosmetic surgery scene, having come into widespread use in the 1990&#8217;s. Tumescent liposuction offers significant benefits over traditional liposuction techniques for its ability to minimize post-operative discomfort, curtail surgical bleeding, and reduce bruising and swelling during the recovery period. </p>
<p>Tumescent liposuction is unique in that it uses large volumes of a dilute solution of lidocaine, paired with the drug epinephrine. The lidocaine serves as a local anesthetic, eliminating the need for patients to go under general anesthesia. The epinephrine serves to temporarily restrict capillaries, thereby reducing bleeding during the surgery. This combination allows surgeons to perform liposuction using minimal amounts of sedatives. The lidocaine is retained in the body for several hours after the procedure, giving the patient up to 12 to18+ hours of pain-free recovery. The use of tumescent anesthetic also eliminates the drunken feeling felt by many patients after going under general anesthesia, as well as the nausea felt by traditional liposuction patients.</p>
<p>At the start of the procedure, the surgeon anesthetizes the skin at select locations using tiny needles. Once the skin has been numbed, longer needles are used to anesthetize the subcutaneous fat with large quantities of the dilute lidocaine and epinephrine solution. Patients are frequently given a mild sedative, such as Valium to help relax them during the procedure. Once the areas to be treated are completely numb, the doctor begins the liposuction. Small holes, called adits, are made in the skin using round skin biopsy punches to form access sites for the liposuction microcannulas. A microcannula is a very small stainless steel tube, ranging from 1mm to 3mm in diameter, designed to effectively remove small amounts of fat. Fat is suctioned out of the body in small tunnels through the microcannula, leaving the fibrous strands attached to the skin and muscles intact. Once the desired fat has been suctioned out, the microcannulas are removed. Since the adits are such small holes, there is no need for sutures using this method. This actually serves to speed up the healing process since drainage can occur via the adits, greatly reducing the amount of swelling and bruising of the treated areas.</p>
<p>Tumescent liposuction, as with any other type of surgical procedure, does carry some risks. Patients should expect some side-effects from the procedure such as bruising, swelling, and temporary numbness of the treated areas. Significant drainage will occur in areas where adits were formed. Some skin irregularities may result from the procedure, though this side-effect is greatly reduced with the use of both the tumescent anesthesia and microcannulas. Other, more serious side-effects can include blood clots, infection, and allergic reactions to the anesthesia.</p>

Local anaesthesia is safer than general anaesthesia and there is a shorter recovery time, with the unwanted side effects of a general anaesthetic persisting for some considerable time following surgery. <p>Laser liposuction is the hottest thing in liposuction that we have seen for a number of years. In the U.S. this has been available for approximately 2 years. Smartlipo was the first on the scene and was FDA approved in 2007. Smartlipo is certainly still the largest in this race but a few newcomers including slimlipo, LipoLite, lipotherme and cool lipo to name a few have come along.</p>
<p>Liposuction has been around for years. Major advances in techniques have occurred. The most significant advances are the development of tumescent technique (injection of numbing solution) and the use of smaller cannulas. Is laser liposuction the next great advancement in liposuction?</p>
<p>What is laser liposuction?</p>
<p>There is a general misunderstanding about laser liposuction. Most advertising would suggest that smartlipo and its competitors have completely reinvented liposuction. In reality all of the basic steps involved with traditional liposuction are performed including:<br />1. Sedation of the patient<br />2. Injection of tumescent fluid (numbing solution)<br />3. Suction of fat.<br />With laser lipolysis there is an additional step which involves the burning of fat just before removal.<br />Initially, the laser was intended to be used without liposuction of the fat. &#8216;Melted&#8217; fat was to be resorbed by the body. This has generally been shown to be ineffective. The fat must be removed after the burning, this is achieved in the EXACT same manner as would be performed without the laser.</p>
<p>The claimed advantages of burning the fat are:</p>
<p>1. Skin tightening<br />2. Less pain<br />3. Quicker recovery<br />4. Smoother results<br />5. Less invasive procedure, some ads even call it fat removal &#8220;without surgery or scars&#8221;<br />6. Quicker procedure</p>
<p>How accurate are these claims?</p>
<p>According the the American Society of Plastic Surgeons, &#8220;The only study that has scientifically compared this to standard liposuction has found no differences between these techniques in terms of the results.&#8221;</p>

<p>&#8220;I was recently given the opportunity to &#8216;try out&#8217; a laser lipo machine on a patient we had scheduled for liposuction. She had liposuction on the abdomen,upper hips, back, and inner thighs. The laser lipo was used on the abdomen to hopefully tighten some loose skin.&#8221;</p>
<p>&#8220;The experience was less than satisfying. There were not any complications due to the laser, however it just didn&#8217;t live up to the hype. Although the other treatment areas were nearly painless, the abdomen (laser treated) was extremely painful and was reported as a &#8216;burning&#8217; sensation. The addition of the laser did not increase the amount of fat that could be removed, nor did I see any skin tightening.&#8221;</p>
<p>&#8220;The only difference I can see between the two procedures is increased pain, increased cost and need for deeper sedation. It is certainly just as invasive as traditional liposuction if not more so. I have researched this extensively and cannot find any ground to support any of the claims that laser lipo is making. Hopefully these studies are done and hopefully the results are favorable for skin shrinkage.&#8221;</p>
<p>Disadvantages of laser liposuction:</p>
<p>The purported advantages of laser liposuction don&#8217;t seem to be accurate. There are, however a few disadvantages of laser liposuction in the authors opinion:</p>
<p>1. Most surgeons (not all) using laser liposuction are not experienced plastic surgeons. &#8220;Cosmetic surgeon&#8221;, is not the same thing and may be an oral surgeon, family practice doctor, OB, radiologist or anyone with a medical degree.<br />2. The burning is painful<br />3. The burning requires a longer procedure with more time and risk. Does it make any sense to burn tissue before you remove it?<br />4. Burning of fat makes it a more (not less) invasive procedure<br />5. The procedure is more expensive (the equipment is very expensive)</p>
<p>How has laser liposuction gotten so popular if it doesn&#8217;t do what it says it does?</p>
<p>Purely marketing genius. Add a laser to anything in medicine and the assumption is that it must be better. This undoubtedly true in Lasik eye surgery and laser hair removal. The bombardment of billboards, TV and print media have made it extremely popular. Many of these ads have been misleading and irresponsible. The claims make laser lipolysis sound like a quick lunch hour procedure. It is simply not the case. Liposuction is one of the most dangerous procedures that plastic surgeons do and all plastic surgeons take it very seriously. The fact that the procedure can be done with light sedation and the incisions are small should not be misinterpreted as minimally invasive. The majority of the deaths (although very rare) in cosmetic surgery are liposuction related.</p>
<p>Many of the doctors using these devices learn to do liposuction over a weekend course, a craft that takes plastic surgeons 6-7 years. The consumer should be very careful. Find the right doctor and together determine what would be your best treatment option. The technology behind the doctor is less important when it comes to liposuction.</p>

<p>82)       <strong>What garment do I wear after Tumescent Liposuction?</strong></p>
<p>Stabilizing your tissues in a special garment increases your comfort and ability to do things. Your tissues will be more comfortable after Tumescent Liposuction when they are not jiggling around. Compression also helps control swelling and bruising. Our patients wear this for about six weeks full time and then part time for 2 more weeks. Different styles are available for the various zones treated by the Tumescent Liposuction. We help fit our patients in an elastic garment specially designed for this surgery. A girdle like pants is used for the buttocks and thighs. An elastic vest is used for the male chest. Hooks and zippers on side openings assist in getting the garments on and taking them off.</p>
<p>83) <strong> Can I gain weight after Tumescent Liposuction surgery?</strong></p>
<p>Tumescent Liposuction removes fat cells from localized parts of the body. Some cells are left behind intentionally. If too much fat is removed, the skin will stick unnaturally to the underlying muscle and tissues. The remaining fat cells can still store unused energy. For those who eat more than they use with exercise, the extra energy will be put away as fat. The distribution of where you put on the fat may be different after Tumescent Liposuction surgery.</p>

<p>Tumescent Liposuction can significantly reduce some of the unpleasant side effects of other techniques. And because Tumescent Liposuction provides local anesthesia, patients can avoid the side effects of general anesthesia, such as vomiting and nausea.</p>
<p>88)       <strong>Can I undergo Tumescent Liposuction for weight loss purposes?</strong></p>
<p>Tumescent Liposuction is not a good way to lose weight. Actually, very little weight is removed by this operation. Suction lipectomy is a technique to sculpt localized collections of fat. It is a tool of refinement. You are better off getting closer to your ideal weight first and then using plastic surgery to refine the resistant localized problem areas. Some doctors are trying to expand the boundaries of safe Tumescent Liposuction by removing massive amounts of fat from very obese people. This large volume Liposuction has very significant risks and can be dangerous despite some excellent results.</p>
<p>89)       <strong>Will Tumescent Liposuction helps me with loose skin?</strong></p>
<p>Tumescent Liposuction does not help loose skin. Suction lipectomy removes extra fat only. When skin has good elasticity, it shrinks nicely after fat sculpture. Excess fat and skin are better managed by operations such as tummy tuck (abdominoplasty), thigh lift, arm lift, and face lift or neck lift. It can be confusing for a patient to figure what type of plastic surgery is best for their problem on their own. <p>90)       <strong>What areas are treated by Tumescent Liposuction?</strong></p>
<p>In women the most frequently areas treated by Tumescent Liposuction are the outer thighs followed by the abdomen. In men the most common area is the flank or &#8220;love handle&#8221; region. Other areas commonly treated include: the face, neck, upper arms, upper and lower abdomen, lower back, hips, thighs inner and outer, sides of the knees, and lower legs. It is uncommon to treat all of these areas at one time (if all areas were involved weight loss may be a better option). Usually there are only a few regions that require Tumescent Liposuction.</p>
<p>91)       <strong>How do I prepare for Tumescent Liposuction surgery?</strong></p>
<p>Tumescent Liposuction preparation requires avoiding medicines that can cause bleeding. Aspirin, Ibuprofen, and many arthritis medications interfere with platelet function and must be stopped 2 weeks before surgery. Many cold, sinus, and other medications contain these drugs and must also be stopped. Vitamin E interferes with the blood clotting factors from Vitamin K. If applicable, stop taking Vitamin E at least 3-4 weeks before surgery. Cigarette smoking can hinder wound healing, so please stop at least 3 weeks before your operation.</p>
<p>To avoid ingrown hairs, don&#8217;t shave or remove hair near the regions to be sculpted the day of surgery. You will not be able to eat after midnight in order to arrive with an empty stomach the morning of surgery. You will be able to take your medications with a small sip of water that morning. Check with your cosmetic surgeon about any medications that you will be taking. Wear loose fitting clothing that opens in the front or pulls over easily; this will make it easier to dress over your pressure garment and dressings.</p>
<p>92)       <strong>How is the recovery time for Tumescent Liposuction?</strong></p>
<p>Tumescent Liposuction leaves local anesthetic solution in the tissues providing greater comfort after surgery than the older techniques. This prolonged relief can be of value in cases done under general anesthesia or sedation. Once this wears off, however, most patients experience some discomfort that is usually easily managed with some oral medication for a few days.</p>
<p>You may weigh more, about the same, or less right after surgery. Some of this is from the extra fluids used during the Tumescent technique that you will urinate over the next day or so. Some weight will be from the fluid of tissue swelling. How much and where depends on what was done. Since there is less bruising and bleeding from Tumescent Liposuction, there is less blood to be absorbed and irritate tissues. The lack of this irritation means that the swelling goes down much faster.</p>
<p>After sedation or general anesthesia, eat lightly at first to avoid an upset stomach. Drinking some extra liquids will help your body adjust its fluid balance. Since there is a lot of fluid injected during Tumescent Liposuction, some does leak out of the tiny wounds after surgery. This is normal and should end within a week. Using feminine pads or dressings to catch this liquid is an effective way to deal with this minor inconvenience. You can resume showering by the second day after surgery.</p>
<p>Tumescent Liposuction results in a rapid improvement of your shape. Most of the swelling goes down within 10 days but part of the result is from the contraction of tissue that has been treated. Although patients see a dramatic improvement within 7-14 days, it will take 3 to 4 months to see most of the changes in your contour.</p>
<p>93)       <strong> When can I resume my work and activities?</strong></p>
<p>You will need to limit your activity for the first couple of days &#8211; no strenuous exercise, lifting or bending over. Avoid any type of aerobic exercise (any activity that raises your pulse above 90) for at least three weeks. That much work may increase your swelling. Many are able to return to work almost immediately. It depends on what you do and the activity level of your job. Resumption of activities also depends on the extent of your Tumescent Liposuction, your health, and how you heal. In general the discomfort lessens significantly in 2-4 days. Stabilizing your tissues in a special girdle (garment) increases your comfort and ability to get around.</p>
<p><strong>ULTRASONIC ASSISTED LIPOSUCTION</strong></p>
<p>94)         <strong>Ultrasonic Assisted Liposuction. What is it?</strong></p>
<p>Ultrasonic Liposuction, or UAL, is a procedure that uses high-pitched sound waves to liquefy fat in a specific area before it is removed. UAL is a type of Liposuction that is primarily used for fat removal in hard-to-treat areas such as the chin, neck, cheeks, knees, calves and ankles. It usually serves as an addition to Tumescent Liposuction for patients who require more precise body contouring.</p>
<p>95)       <strong>How do I know whether or not I am a candidate for Ultrasonic Liposculpture?</strong></p>
<p>Traditional Tumescent Liposuction has been very effective in young patients with mild to moderate amounts of fat. Ultrasonic Liposuction expanded the number of patients suitable for body sculpting. This includes patients with larger amounts of fat and less elastic skin tone. It also allows surgeons to treat areas not easily or effectively treated in past years. This would include fibrous areas such as the back, male breast and upper abdomen. A physical examination is required in order to verify whether you are a candidate for this procedure.</p>
<p>96)       <strong>What medical conditions might exclude me from being a candidate?</strong></p>
<p> A patient needs to be in good medical condition in order to undergo this or other elective procedures. A patient should have the ability to heal incisions and adequately fight infections, and no medical contraindications such as a heart or lung disease. Heavier patients are now candidates for this type of procedure; however, obese patients with medical contraindications are not candidates. Patients with excessive amounts of redundant skin are not candidates for this procedure.</p>
<p>97)       <strong>What sets Ultrasonic Liposuction apart from traditional Tumescent Liposuction?</strong></p>
<p> What makes this so unique is the ability to gently liquefy or emulsify the fat without injuring the adjacent structures. This is a non-mechanical method of dissolving the fat that also appears to tighten the skin. There has been less blood loss, less bruising and decreased tissue trauma observed. Ultrasonic Liposuction allows us to also removed fat in areas that were preciously unable to have Liposuction such as the back, upper abdomen and male breast.</p>
<p>98)       <strong>What is Internal Ultrasonic Liposuction?</strong></p>
<p>Ultrasonic Liposuction, (Ultrasound Assisted Lipoplasty) or &#8220;Ultrasound Assisted Liposuction&#8221; (U.A.L.) is a technique of Liposuction where the fatty tissue is removed with the assistance of sound waves which are beyond the range of human hearing. Because the sound waves are doing the work the surgeon is able to more precisely guide the cannula. This increases safety. The Ultrasonic energy is targeted to fat cells and creates less damage to other tissues such as nerves, blood vessels and muscles.</p>
<p>99)       <strong>What Areas of the Body Does UAL Treat?</strong></p>
<p>Ultrasonic Liposuction can be used in all areas that traditional Liposuction treats, but it works especially well on areas that require more precision. The stomach, midsection or “love handles,” male breast tissue, chin, and neck all respond well to UAL, as do areas that have been previously treated with Liposuction but are in need of further contouring.</p>
<p>100)     <strong>How can the plastic surgeon see what his doing?</strong></p>
<p>The Ultrasonic Liposuction equipment is highly sophisticated. The fat is dissolved by the Ultrasonic waves that are emitted from the end of a cannula. This liquefied fat is then gently aspirated away. While endoscopic techniques have been utilized to inspect the areas in which the fat has been removed, it is not necessary on a routine basis. This is because the results are immediately visible, just as when ice or snow melts in the sun.</p>
<p>101)     <strong>How do I decide whether to have Liposuction-Liposculpture, or Ultrasonic Liposuction?</strong></p>
<p> The surgeon should review his experience with the various methods and analyze your body type. Some surgeons may advise Ultrasonic Liposuction for large volume extractions, especially in tough areas such as the back, flanks and hips. It was designed principally for the hard to extract deep fat and to reduce surgeon fatigue. External Ultrasonic energy can be applied and is recommended by some surgeons before or after the fat is removed by regular Liposuction. Ultrasonic Liposuction must ordinarily be combined with traditional Liposuction when deep and surface fat is being removed. This does add to the operating time or to the cost of the surgery and may offer no advantage for most surgeons. For gynecomastia (large breasts in the male), some experts advocate the sole use of Ultrasonic Liposuction but it should not be used aggressively near the nipple skin.</p>
<p>102)     <strong>How the Ultrasonic Liposuction Procedure is done?</strong></p>
